                A note about gift cards:

When purchasing a gift certificate through online check out, you will be given the option to have the certificate e-mailed to the recipient. In addition to e-mail delivery, if you would like a specialty designed RelaxAmerica Massage gift card to be mailed to you or mailed directly to your recipient please fill out the gift card request form below.

If you would prefer to only have it mailed, do not fill in the e-mail address recipient field during online checkout.  Once purchased, return to this form and be sure to type in your online gift card verification number.
